As a primary care practice, we perform annual exams & Welcome to Medicare exams.
Immigration Physical Exams
Please download form I-693 from www.uscis.com. Fill out and bring to your appointment.
School & Sports Physicals
A sports physical, also known as as pre-participation physical examination (PPE), helps to determine whether or not a child is physically able to safetly participate in a sport.
Primary Care Services
We offer a comprehensive range of primary care services including regular checkups, immunizations and treating the following conditions: colds and flus, skin conditions, respiratory issues, neurological problems, musclo-skeletal issues, gastro intestinal conditions, chronic conditions, and heart health including EKGs onsite.
Same Day / Urgent Care
Because illness hits you when least convenient, we offer urgent same-day visits.
Pre-Employment Physical Exams
We currently offer pre-employment physical exams for Hackettstown fire Department & House of the Good Shepard.
Diagnostic Testing
Our comprehensive diagnostic testing includes lab work, drug testing, immunizations & EKG’s.
Geriatric Care
We have a large geriatric patient population. Dr. Kavathia works closely with patients, their families, visiting nurses, hospice care services, and nursing homes to coordinate care for our elderly patients.
